ServSafe® Food Protection Manager Certification Course – May 19th

The Columbus Department of Public Health is proud to sponsor on May 19th,  an 8-hour ServSafe® Food Protection Manager Certification Course. This course provides training that prepares you for ServSafe certification and re-certification as a Certified Food Protection Manager.  2016 May 19 ServSafe 8 hour Course Registration Form

Sponsor/Location: Columbus Department of Public Health – (2nd Floor – LDR Classroom) – Time: 8:30 a.m. – 5:00 p.m. Price: $150.00 (Non-refundable)

NOTE: Class size is limited. Early registration is recommended. Attendee is enrolled when payment is received. If not available at time of registration, the course book will be mailed to the attendee’s address at least one week prior to the class. Cancellation of this course is required 72 hours prior to the start of the class in order to remain eligible for registration. Failure to notify this office 72 hours prior to the course date that you will not be attending this course will result in a $30 fee payable prior to rescheduling. The attendee’s registration is valid for 12 months from date of registration. At the end of the 12 months, if the attendee has not yet taken the class, they must re-register and re-pay the entire amount. There are NO refunds. Course dates are subject to change. Attendee will be contacted in advance if date is changed.
