LOCATION: Columbus Department of Public Health, District Child Health/Children’s Medical Services, 5601 Veterans Parkway, Columbus, GA 31904

DUTIES: Under general supervision, this position will provide guidance and oversight of the Columbus Children’s Medical Services program and will provide care coordination and case management to infants and children 0 – 21 years of age. Directs and supervises programmatic activities and clinical staff to ensure provision of effective, efficient and safe clinical care while providing nursing care coordination consistent with the patient’s level of need and in accordance with policies and procedures. Collaborates with other health care providers and community agencies to provide clinical and population-based services. This position will be responsible for monitoring clinic activity, schedules, and clinic/program supplies. Must be able to identify training needs and ensure training is provided.

MINIMUM QUALIFICATIONS: Bachelor’s degree in nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate’s degree in nursing from an accredited college or university AND One year of nursing experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.

PREFERRED QUALIFICATIONS: Professional experience in public health nursing and completion of a Bachelor’s Degree in nursing.
