Social Worker, Non-Licensed 1 (Case Manager)

PAY GRADE: G District Entry/Hiring Salary: $27,248.75/year
Salary based on education and experience

LOCATION: District Clinical Services, Columbus Health Department, 5601 Veterans Parkway, Columbus, GA 31904

DUTIES: Under general supervision, provides persons, families, and/or vulnerable populations with the psychosocial support needed to cope with chronic, acute, or terminal illnesses and physical or developmental disabilities. Assesses, screens and counsels clients in a variety of program areas performing social services activities to enhance, meet or restore their functioning capacity. Develops or participates in case management/care plans. Provides patient education and counseling, advises caregivers, and makes necessary referrals for other social services. This position will work with patients in an infectious disease clinical setting.

ENTRY QUALIFICATIONS: Master’s Degree in a related field from an accredited college or university AND eligibility to be licensed OR Bachelor’s degree in a related field and Two (2) years of experience in social service delivery. Copy of college transcript is required when applying.

PREFERRED QUALIFICATIONS: Master’s degree in social work, psychology, counseling, sociology, education, nursing, vocational rehabilitation, or a related field and two (2) years of full-time (or equivalent part-time) work experience in social services delivery (case management, outreach, prevention/education, client assessment, or the development / implementation of treatment, rehabilitation, or service plans).
