SALARY: $66,168.17 Annually
LOCATION: Children’s Medical Services, Columbus Health Department, 5601 Veterans Pkwy, Columbus, GA 31904
DUTIES: Under general supervision, this position will provide guidance and oversight of the Columbus CMS (Children’s Medical Services) program and will provide care coordination and case management to infants and children 0 – 21 years of age. Directs and supervises programmatic activities and clinical staff to ensure provision of effective, efficient, and safe clinical care while providing nursing care coordination consistent with the patient’s level of need and in accordance with policies and procedures. Collaborates with other health care providers and community agencies to provide clinical and population-based services. This position will be responsible for monitoring clinic activity, schedules, and clinic/program supplies. Must be able to identify training needs and ensure training is provided.
MINIMUM QUALIFICATIONS: Master’s degree in nursing, public health or related field from an accredited college or university AND Two years of nursing experience AND current Georgia Registered Professional Nurse license AND CPR certified OR Bachelor’s degree nursing from an accredited college or university AND Five years of nursing experience AND current Georgia Registered Professional Nurse license AND CPR certified OR Two years of experience at the lower level PH Nursing Prog Consultant 1 (HCP210) or position equivalent AND current Georgia Registered Professional Nurse license AND CPR certification.
PREFERRED QUALIFICATIONS: Public Health Nursing experience, leadership/supervisory experience; experience with coordinating efforts and/or collaborating with local community organizations, health care providers, etc.
