Public Health Registered Nurse 1 (1st Care Program)

PAY GRADE: $51,811.56 Annually

LOCATION: 1st Care/Plan of Support & Care, Columbus Health Department, 5601 Veterans Pkwy., Columbus, GA 31904

DUTIES: Under supervision, this position will provide case management and nursing assessments to families that qualify for the 1st Care/Plan of Support and Care Program and postpartum moms throughout Muscogee, Sumter, Macon, and Crisp counties. This position will perform in-home comprehensive assessments of client’s health care needs & developmental screenings within the guidelines. Develop comprehensive plans of care for clients and provides case management to patients/families consistent with the plan of care, program deliverables, and District policies and procedures. Will provide developmental screenings, patient education & counseling, advise caregivers and make necessary referrals for other social services. May also assist with newborn hearing screenings, as needed.

MINIMUM QUALIFICATIONS: Associate’s degree in nursing from an accredited college or university AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification. No experience required. *A copy of college transcripts is required.

PREFERRED QUALIFICATIONS: Completion of a Bachelor’s degree in Nursing and one (1) year of experience as a Registered Professional Nurse.

All West Central Health District 7 employees are deemed essential workers in the event of an emergency.
