LOCATION: Children’s Medical Services, Crisp County Health Department, 111 E. 24th Ave., Cordele, GA 31015
DUTIES: Under supervision, this position will travel throughout the Crisp, Dooly, Macon, Marion, Schley, Sumter, Taylor, and Webster County health departments to provide case management and nursing assessments to families that qualify for the 1st Care/CMS (Children’s Medical Services) program. This position will perform in-home comprehensive assessments of client’s health care needs & developmental screenings within the guidelines. Develop comprehensive plans of care for clients and provides case management to patients/families consistent with the plan of care, program deliverables, and District policies and procedures. Will provide developmental screenings, patient education & counseling, advise caregivers and make necessary referrals for other social services. Will also serve as liaison between families & vendors and, assist with transitioning families from pediatric to adult health care, and participate in orthopedic clinics.
MINIMUM QUALIFICATIONS: Associate’s degree in nursing from an accredited college or university AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification. No experience required. *A copy of college transcripts is required.
PREFERRED QUALIFICATIONS: Completion of a Bachelor’s degree in Nursing and one (1) year of experience as a Registered Professional Nurse.
