DUTIES: Under supervision of the Perinatal Home Visiting Program Coordinator, this position will be housed in one of the following county health departments (Crisp, Dooly, Macon, Randolph, Stewart, or Sumter) and is Not a Remote position. Daily travel is a requirement for this position. This position will provide professional nursing care in accordance with nurse protocols, policies, and/or procedures to include, but not limited to:
•Provide home visitation services to medically diagnosed high-risk, pregnant, post-partum and inter-conceptual women.
•Works with a team to provide case management services to high-risk perinatal women, infants, and children through home visitation.
•Completes quarterly reports
•Participates in health fairs and educational programs
•Incorporates strategies for interacting with persons from diverse backgrounds (e.g., cultural, socioeconomic, educational, racial, gender, age, ethnic, sexual orientation, professional, religious affiliation, mental and physical capabilities
•Collaborate with healthcare professionals and community partners to provide person-centered support to individuals and families.
•Participate in emergency preparedness and response activities.
•Candidate must have a valid, unexpired driver’s license and reliable transportation
•Travel throughout the district’s 16 counties for training and program support, as necessary
MINIMUM QUALIFICATIONS:
Public Health LPN 2 – (Hire-in salary – $35,387.99)
Vocational/Technical degree in licensed practical nursing from an accredited college and two (2) years of LPN public health experience and current Georgia license to practice as a Licensed Practical Nurse and CPR certification; or one (1) year of experience required at the lower-level PH LPN 1 (HCT110) or position equivalent.
Public Health LPN 3 – (Hire-in salary – $38,926.79)
Vocational/Technical degree in licensed practical nursing from an accredited college and four (4) years of LPN public health experience and current Georgia license to practice as a Licensed Practical Nurse and CPR certification; or one (1) year of experience required at the lower-level PH LPN 2 (HCT111) or position equivalent.
PREFERRED QUALIFICATIONS: At least four (4) years of experience as a Licensed Practical Nurse. Public Health experience.
All West Central Health District 7 employees are deemed essential workers in the event of an emergency. As such, you may be called on to participate in an event including but not limited to: staffing a shelter, working at a Specimen Point of Collection Site (SPOC) and/or working at a Point of Dispensing site (POD).
