First Care / High Risk Infant Follow UpBaby 2

First Care is a voluntary program that offers nursing assessments, developmental evaluations, and case management to children from birth to one year of age who may be at risk for developmental delays, whether or not they have a medical condition. Services are provided via phone, home visits, or at the Health Department.

In addition, First Care is a good resource for families in need of medical equipment, a therapist, pediatrician, or a specialist. The program works in collaboration with all health care providers, social services, as well as other community agencies.

First Care is available in:  Clay, Crisp, Dooly, Macon, Marion, Randolph, Schley, Sumter, Taylor, and Webster counties.
