The DPH Home Visiting Program provides home visiting services for women during pregnancy and after delivery until your baby has their first birthday. Pregnant women with high-risk conditions, or who have risk factors that put them at risk will be the focus for home visiting services, but enrollment is accepted after delivery. These services are available for women in the following counties: Crisp, Dooly, Harris, Macon, Randolph, Stewart, Sumter, Talbot, and Taylor.
The program provides additional monitoring between appointments with your healthcare provider which helps to identify warning signs, complications, and problems as soon as possible. This allows women to receive further assessment and intervention when indicated.
Some of the services provided include:
Clinical assessment of mother and baby
Care coordination
Case management
Education for high-risk pregnant women and their families
Referrals to other needed resources
Below are some conditions that can be referred to the program:
Maternal patients with:
- Hypertension or Gestational Hypertension
- Preeclampsia
- Diabetes
- Multiple gestation
- Prior preterm delivery
- Preterm labor
- Chronic conditions or co-morbidities
- Prior 2nd trimester pregnancy loss
- Prior fetal/neonatal death
- Pre-existing health conditions
- Substance use disorder
- Mental health condition
- Poor support system
- Difficulty complying with provider recommendations and/or follow up (e.g., keeping appointments)
- Other medical conditions or concerns for poor outcomes
Home Visiting Staff
Infants with:
- Recent NICU discharge
- Very Low Birth Weight or Low Birth Weight
- Less than 36 weeks gestation at delivery
- Positive maternal screening for substances at delivery
- Suspected or confirmed congenital syphilis or HIV infection
- Poor maternal support system or other environmental concerns
